To have medical records transferred please send an email to mail@animalcarecenter.org with client name, animals’ names, and the name and fax number of the receiving hospital.  This email will function for at least three years for the purpose of receiving records transfer requests.  Records will be faxed within 10 days of the receipt of the request.
